The medical team clears a patient for discharge. The paperwork is ready. The prescriptions are written. Then someone asks the question that stops everything: where will this person go tonight?
If the answer is unclear, the whole discharge slows down. Discharge planners make call after call. Families worry about what comes next. The patient waits in a hospital bed, ready to recover somewhere else but with nowhere safe to go. Meanwhile, the risk grows. A patient who leaves without a safe place to recover often struggles to take medications, attend follow-up visits, and rebuild strength.
Here is the good news. Most of these delays are preventable. They usually trace back to three causes: a late start, missing information, and unclear next steps. You can fix all three, and you can fix them before discharge day arrives.
This guide shows you how. You will learn when to reach out, what to confirm, and what to include in a complete referral. You will also see how Hazel’s Tranquility Place and our sister agency, Solano Impact Care, help you move faster. Our program serves adults across Solano County who need structured, supportive housing during a vulnerable transition, including adults leaving the hospital.

Start the Housing Conversation Early
Late contact causes most delays. Many teams wait until the discharge date is firm. By then, options have narrowed.
Instead, reach out as soon as you know a patient lacks a safe place to recover. Do not wait for perfect information. Start the conversation first. Then fill in the details together.
Early contact gives our team time to review the referral. It also gives you a clearer picture of what is available.

Confirm the Patient Fits Admission Requirements
Fit questions slow referrals down. Check the basics before you call. That way, your inquiry moves straight to availability.
Hazel’s Tranquility Place lists these admission requirements:
- The patient must be alert, oriented, and able to engage with an intake specialist.
- The patient must self-administer medications independently.
- The patient must be independent with hygiene, unless arriving with a formally assigned IHSS support person.
- Oxygen patients must manage a personal portable unit independently.
- The patient must have required aftercare established, such as wound care, physical therapy, home health, or IHSS.
- The patient must be ready for clean and sober living.
Referrals also receive screening for substance use considerations. Clients who need detox or stabilization may receive a referral to a higher level of care before placement.

Submit a Complete Referral the First Time
Incomplete referrals create back-and-forth. Each follow-up call costs time. Therefore, send a complete picture from the start.
You can submit a referral in three ways:
- By email at care@solanoimpactcare.org
- By phone at 707-301-4051
Next, add important notes. List every patient barrier you know. This step helps the team identify needs. It also shows whether the patient could benefit from other services, even if a bed is not the right fit.
Additionally, share the patient’s mobility and daily living needs. Our assessment covers mobility, activities of daily living, health and wellness, and independence. Complete details speed up that review.

Raise Insurance Questions Early
Insurance uncertainty stalls many referrals. However, you do not need every answer before you call. Bring the question into the conversation.
Hazel’s Tranquility Place is credentialed by Partnership HealthPlan, Medi-Cal, and Kaiser Permanente. Placement is free for qualifying members of Partnership HealthPlan of California and Kaiser Permanente.
If coverage is unclear, tell our team what you know. They will help you work through the details. Meanwhile, the review keeps moving.
Plan for the 48-Hour Turnaround
Solano Impact Care provides a 48-hour turnaround for referrals. Build that window into your discharge timeline.
Here is how the process works:
- You submit the referral.
- Solano Impact Care receives it and verifies eligibility.
- An intake coordinator meeting takes place.
- The team issues an approval or recommends alternatives.
Timing matters. If your discharge date is close, call 707-301-4051 and ask about expedited placement. Do not rely on a web form alone when the clock is tight.

Ask About Services Beyond a Bed
Sometimes a bed is not available right away. Sometimes a patient does not meet bed criteria. Either way, the referral still has value.
Solano Impact Care evaluates every referral for all available services. Those services include:
- Enhanced Care Management: A dedicated care manager helps enroll patients with a primary care physician and coordinates medical needs. It is typically authorized for up to 12 months through Partnership HealthPlan or Kaiser, and it is available regardless of bed placement.
- Housing Navigation: Staff work with each person to identify income, realistic housing options, roommate situations, waitlists, and applications.
- Housing Sustainability: This post-placement program covers budgeting, tenant rights, and landlord mediation.
- Resource connections: Staff link patients to partner organizations for whole-person health and wellness.
So, even a “not right now” on a bed can still open the door to real support.
Prepare the Practical Details
Small logistics cause big delays. Plan for them before discharge day.
Consider these points:
- Transportation. Partnership HealthPlan covers transportation to medical appointments. The women’s program in Cordelia offers Fairfield Fast Connect, a door-to-door shuttle for non-medical trips. The men’s program in Vallejo sits near a SolTrans bus stop. Residents can also use Uber or rideshare independently.
- Accessibility. The women’s house is entirely ground level. The men’s house has most rooms upstairs, with one downstairs bedroom. Both facilities have walk-in showers with handrails and grab bars next to toilets.
- Outside providers. Physical therapists, wound care nurses, and similar providers may visit residents on-site. Confirm those arrangements ahead of time.
- Food. Meal service is available but optional. Staff can help residents enroll in EBT.
These details help your team confirm fit before the patient arrives.
Keep Communication Moving
Silence slows everything. After you submit a referral, stay reachable. Answer follow-up questions quickly. Confirm decisions promptly.
In addition, name one lead contact for housing on your team. That person should track the referral. Otherwise, hospital staff, case managers, and families may each assume someone else is handling it.
Our article on how case managers coordinate with Hazel’s Tranquility Place covers how to build that coordination.
